Patient Intake Process
1) Free Consultation (15 min)
You can book a Free Consultation where you can explain your symptoms and get feedback on what we may be able to do for you. Depending on your preference, this free consultation can be by phone or online. Based on what you learn during this free consultation, you can decide whether or not to proceed with treatment. There is no obligation.
2) Initial Intake Appointment
If you decide to proceed with treatment, we will book an Initial Intake Appointment for you and send three (3) Intake Forms to you by email. We ask that you fill and return them to us by email, before your appointment. The Intake Forms are,

Form consenting to the collection of health data
We ask that you return the filled forms at your earliest, allowing time for the practitioner to review your health history and prepare beforehand for the appointment. During the Initial Intake appointment, the practitioner will go through your health history with you, by asking questions in order to gather information relevant to your condition and treatment. The practitioner will propose a treatment plan based on the information gathered. We may also be able to fit-in a short acupuncture session if time permits.
3) Follow-up Appointments
Follow-up Appointments will be dedicated to treatment proposed in the treatment plan. The practitioner will seek feedback from you in order to monitor your progress.
